Life After Pterygium Surgery: What Recovery Really Looks Like

The question almost every patient asks before surgery isn’t about the procedure itself. It’s the one that comes right after: how long until I’m back to normal?

It’s a reasonable thing to want to know. Life doesn’t pause for recovery. There’s work, family, commitments — and for most people in the 30 to 60 age bracket, the idea of extended downtime is as daunting as the surgery itself. So here’s an honest, week-by-week account of what recovery from pterygium surgery actually involves.

The first 24 hours:

The anaesthetic block used during surgery continues to provide comfort for several hours afterwards. When it wears off, some mild to moderate discomfort is normal and prescribed pain relief manages this effectively for the vast majority of patients. Plan to rest at home. This is not the afternoon to push through.

Your eye will be padded or shielded when you leave the surgery, and you’ll have a follow-up appointment with Dr Pappalardo the very next day to check healing progress and comfort. That appointment matters…. don’t skip it.

The first week:

Redness and a gritty sensation are the two things patients mention most in the first week. Both are a normal part of healing. Neither means something has gone wrong. Reading, screen time, and driving may be uncomfortable in the early days; not because they’re harmful, but because the eye is still settling. Most day-to-day activities can resume as your comfort allows. Light tasks around the house are fine. Intense exercise and swimming are not — both are off the table for the first month.

Your drop regime begins immediately after surgery and is essential to follow precisely:

  • Antibiotic drops for the first week, to prevent infection
  • Anti-inflammatory drops for approximately six weeks, to optimise healing
  • Lubricating drops as needed throughout recovery, for comfort

These aren’t optional extras. The drops are a core part of the surgical outcome.

Weeks two to four:

This is where most patients start to feel like themselves again. Comfort improves steadily, the appearance of the eye settles considerably, and by weeks three to four the majority of patients describe feeling broadly back to normal.

The eye will still look red — sometimes more than patients expect — but this continues to improve. The final cosmetic result takes longer to fully emerge, and that’s worth understanding before surgery so the healing process doesn’t feel alarming.

One thing that occasionally surprises patients: some experience brief episodes of double vision when looking away from the surgery side in the early weeks. This is a known temporary effect and typically resolves within days to two weeks. Mention it at your follow-up if it persists.

The longer view:

The four-week appointment with Dr Pappalardo is where healing is formally assessed and medications are reviewed. Beyond that, additional visits are scheduled based on individual progress. What most patients don’t fully appreciate until they’re on the other side of it is that the eye continues improving for several months after surgery. The final cosmetic outcome — the result that makes most people forget which eye was operated on — is typically fully apparent by the three-month mark.

Recovery is a process, not an event. The patients who do best are the ones who follow their drop regime carefully, protect their eye outdoors with quality wraparound sunglasses, and keep their eyes covered during bathing for the first two weeks.

Ready to take the next step?

If you’re weighing up whether the recovery is manageable alongside your life, the honest answer for most patients is yes — with some planning. A referral from your GP or optometrist is the starting point, and your consultation with Dr Pappalardo is the right place to ask every question specific to your situation.
